Indonesia releases conclusions of fatal 2021 Sriwijaya Air accident investigation

Indonesia's National Transportation Safety Committee, or Komite Nasional Keselamatan Transportasi (KNKT), has issued its final report on the January 9, 2021 Sriwijaya Air 737-500 accident that killed all 62 people on board. Read More » The aircraft, registration PK-CLC, crashed into the Java Sea shortly after taking off from Jakarta Soekarno-Hatta Airport. The KNKT report said a problem with the aircraft's auto throttle system had been reported 65 times since 2013 and was an issue on the fatal flight. Pilots were unable to recover the situation after the aircraft entered an upset condition, the report said. Inadequate upset prevention and recovery training contributed to the inability of the cockpit crew to prevent and recover from the upset condition, it concluded.
